Filing an Insurance Claim

To submit a claim to the City of Jackson, please complete the attached form and submit it to the City Clerk's Office, 161 Michigan Avenue, Jackson, Michigan  49201.  The form may be faxed to:  1-866-971-2147.

Once submitted, your claim will be forwarded to the City of Jackson's Insurance Carrier for processing and handling.
